Losing Weight with GLP-1 Medications? Don’t Forget Your Muscles

In the world of medical weight management, GLP-1 receptor agonists (like Semaglutide and Liraglutide) have become a game-changer. These medications are incredibly effective at helping people lose significant amounts of body fat, but a growing body of research is highlighting a critical factor for long-term health: what happens to our muscle mass?

A recent systematic review and meta-analysis published in the International Journal of Obesity (2026) took a deep dive into how these "obesity-dose" medications impact body composition. Here is what you need to know if you are currently using or considering these treatments.

The Good News: Better Body Composition

The study found that, overall, these medications significantly improve body composition. This is because they are so effective at burning fat that "lean mass" (which includes muscle) actually makes up a larger percentage of your total weight than it did before. Essentially, your ratio of muscle to fat improves.

The Challenge: The "Absolute" Muscle Loss

While the proportion of muscle improves, the study also confirmed that absolute lean mass decreases. When you lose weight rapidly (whether through surgery, extreme dieting, or medication) your body doesn't just lose fat; it loses muscle, too.

Key findings included:

  • Average Lean Mass Loss: Across the studies, participants lost an average of about 1.74 kg (nearly 4 lbs) of lean mass.
  • The Semaglutide Effect: Medications like Semaglutide showed more significant weight loss, but also higher absolute lean mass loss (averaging 5.44 kg in some trials).
  • Why It Matters: Lean mass isn't just for looks. It is the engine of your metabolism, the framework for your mobility, and the primary protector against falls and fractures as we age.

Avoiding "Sarcopenic Obesity"

The researchers warned of a condition called sarcopenic obesity, where a person has high body fat but dangerously low muscle mass. This combination is linked to poor metabolic control and a higher risk of physical disability.

The study’s conclusion is clear: Drug treatment alone is not enough. To achieve the best clinical outcomes and maintain a healthy metabolism, these medications must be accompanied by specific physical exercise interventions.

How Our Exercise Physiology Services Can Help

If you are using GLP-1 medications, our Accredited Exercise Physiologists (AEPs) are here to ensure you lose the right kind of weight. We specialise in designing targeted resistance training programs and metabolic conditioning specifically tailored to protect your muscle tissue during rapid weight loss.

We don't just give you a workout; we provide evidence-based strategies to:

  1. Preserve Lean Muscle: Offsetting the absolute loss identified in recent research.
  2. Boost Basal Metabolism: Ensuring your weight loss is sustainable long-term.
  3. Improve Functional Strength: So you don't just look lighter, but feel stronger and more capable.
